- x86-64-calgary-get-rid-of-translate_phb.patch removed from -mm tree

The patch titled
     x86-64: Calgary - get rid of translate_phb
has been removed from the -mm tree.  Its filename was
     x86-64-calgary-get-rid-of-translate_phb.patch

This patch was dropped because it was merged into mainline or a subsystem tree

Now that we check for translation enabled/disabled based on the presence of
the IOMMU translation table, we can get rid of translate_phb.
