- i386-remove-volatile-in-apicc.patch removed from -mm tree

The patch titled
     i386: remove volatile in apic.c
has been removed from the -mm tree.  Its filename was
     i386-remove-volatile-in-apicc.patch

This patch was dropped because it was merged into mainline or a subsystem tree

Remove the volatile in apic.  We have a cpu_relax() in the wait loop.  Fix a
coding style issue while at it.
