- no-longer-include-asm-kdebugh.patch removed from -mm tree

The patch titled
     no longer #include <asm/kdebug.h>
has been removed from the -mm tree.  Its filename was
     no-longer-include-asm-kdebugh.patch

This patch was dropped because it was merged into mainline or a subsystem tree

Include the new linux/kdebug.h instead of asm/kdebug.h.

Simply remove the asm/kdebug.h include if both had been included.
