- lguest-makefile.patch removed from -mm tree

The patch titled
     lguest: Makefile
has been removed from the -mm tree.  Its filename was
     lguest-makefile.patch

This patch was dropped because an updated version will be merged

Finally, we put in the Makefile, so it will build.

Linking the switcher code (hypervisor.S) ready to be copied into the top of
memory is the only non-trivial thing here.
