The patch titled
     KVM: MMU: If emulating an instruction fails, try unprotecting the page
has been removed from the -mm tree.  Its filename was
     kvm-mmu-if-emulating-an-instruction-fails-try-unprotecting-the-page.patch

This patch was dropped because it was merged into mainline or a subsystem tree

A page table may have been recycled into a regular page, and so any
instruction can be executed on it.  Unprotect the page and let the cpu do its
thing.
