The patch titled
     KVM: make set_cr3() and tlb flushing arch operations
has been removed from the -mm tree.  Its filename was
     kvm-make-set_cr3-and-tlb-flushing-arch-operations.patch

This patch was dropped because it was folded into kvm-userspace-interface.patch
