The quilt patch titled
     Subject: mm-ksm-document-smart-scan-mode-fix
has been removed from the -mm tree.  Its filename was
     mm-ksm-document-smart-scan-mode-fix.patch

This patch was dropped because it was folded into mm-ksm-document-smart-scan-mode.patch
