[merged mm-stable] zram-make-zram_bio_discard-more-self-contained.patch removed from -mm tree

The quilt patch titled
     Subject: zram: make zram_bio_discard more self-contained
has been removed from the -mm tree.  Its filename was
     zram-make-zram_bio_discard-more-self-contained.patch

This patch was dropped because it was merged into the mm-stable branch
of git://git.kernel.org/pub/scm/linux/kernel/git/akpm/mm

Derive the index and offset variables inside the function, and complete
the bio directly in preparation for cleaning up the I/O path.
