- grab-swap-token-reordered.patch removed from -mm tree

The patch titled
     grab swap token reordered
has been removed from the -mm tree.  Its filename was
     grab-swap-token-reordered.patch

This patch was dropped because it was merged into mainline or a subsystem tree

Make sure the contention for the token happens _before_ any read-in and
kicks the swap-token algo only when the VM is under pressure.
