The quilt patch titled
     Subject: mm-memory-failure-convert-to-pr_fmt-v2
has been removed from the -mm tree.  Its filename was
     mm-memory-failure-convert-to-pr_fmt-v2.patch

This patch was dropped because it had testing failures
