Subject: ia64: simplify code flow around swiotlb init

Before the change CONFIG_INTEL_IOMMU && !CONFIG_SWIOTLB && !CONFIG_FLATMEM
could skip `set_max_mapnr(max_low_pfn);` if iommu is not present on
system.
