[merged] maccess-update-the-top-of-file-comment.patch removed from -mm tree

The patch titled
     Subject: maccess: update the top of file comment
has been removed from the -mm tree.  Its filename was
     maccess-update-the-top-of-file-comment.patch

This patch was dropped because it was merged into mainline or a subsystem tree
