[merged] coda-move-internal-defs-out-of-include-linux.patch removed from -mm tree

The patch titled
     Subject: coda: move internal defs out of include/linux/ [ver #2]
has been removed from the -mm tree.  Its filename was
     coda-move-internal-defs-out-of-include-linux.patch

This patch was dropped because it was merged into mainline or a subsystem tree

Move include/linux/coda_psdev.h to fs/coda/ as there's nothing else that
uses it.
