[merged] slab-make-size_index_elem-unsigned-int.patch removed from -mm tree

The patch titled
     Subject: slab: make size_index_elem() unsigned int
has been removed from the -mm tree.  Its filename was
     slab-make-size_index_elem-unsigned-int.patch

This patch was dropped because it was merged into mainline or a subsystem tree

Subject: slab: make size_index_elem() unsigned int

size_index_elem() always works with small sizes (kmalloc caches are 32-bit)
and returns small indexes.
