The patch titled
     Subject: dax: add tracepoints to dax_pfn_mkwrite()
has been removed from the -mm tree.  Its filename was
     dax-add-tracepoints-to-dax_pfn_mkwrite.patch

This patch was dropped because it was merged into mainline or a subsystem tree

Subject: dax: add tracepoints to dax_pfn_mkwrite()

Add tracepoints to dax_pfn_mkwrite(), following the same logging
conventions as the rest of DAX.

Here is an example PTE fault followed by a pfn_mkwrite:
