The patch titled
     Subject: dax-enable-iostat-for-read-write-fix
has been removed from the -mm tree.  Its filename was
     dax-enable-iostat-for-read-write-fix.patch

This patch was dropped because it was folded into dax-enable-iostat-for-read-write.patch
