The patch titled
     Subject: mm-introduce-vm_lockonfault-v9
has been removed from the -mm tree.  Its filename was
     mm-introduce-vm_lockonfault-v9.patch

This patch was dropped because it was folded into mm-introduce-vm_lockonfault.patch
