- dm-create-error-table.patch removed from -mm tree

The patch titled

     dm: create error table

has been removed from the -mm tree.  Its filename is

     dm-create-error-table.patch

This patch was dropped because it was merged into mainline or a subsystem tree

Add a library function dm_create_error_table() to create a table that rejects
any I/O sent to a device with EIO.
