- mm-cleanup-swap-unused-warning.patch removed from -mm tree

The patch titled

     mm: fix swap unused warning

has been removed from the -mm tree.  Its filename is

     mm-cleanup-swap-unused-warning.patch

This patch was dropped because it was merged into mainline or a subsystem tree

Convert defines in swap.h into blank inline functions to fix this warning
and be consistent.
