[PATCH 1/3] mips:powertv: Base files for Cisco Powertv platform, v3

Adds the C and header source files for the Cisco PowerTV platform.

Thanks to David Daney and, especially, to Paul Gortmaker for their comments.

History
v3	Added ioremap.h file. Removed usage of MIPS_MT as it doesn't apply
	to the 24K processor. Support PowerTV-specific 48-bit clocksource.
	Moved CEVT_POWERTV and CSRC_POWERTV to arch/mips/powertv/Kconfig from
	arch/mips/Kconfig. Split out ASIC- and model-dependent files. Many,
	many style changes to conform to kernel code style.
v2	Check clocksource function to correspond to changed prototype.
