It appears to 'drive' a proper Skype desktop client from a SIP interface - so your regular SIP phone (IT, desk phone, anything) will connect to SipToSis using SIP, then that will relay to Skype. I think you can do this without Asterisk, you just need to be running the relay somewhere.
