[RFC v2 10/10] nvme: enable LBS support

Now that the block device cache allows LBS, and we have buffered-io
support, and we support both buffer-heads and iomap aops on the
block device cache through the inode dynamically, enable support for
LBA formats > PAGE_SIZE.
