[PATCH] orinoco: checkpatch cleanup

Subject: [PATCH] orinoco: checkpatch cleanup

Fix errors and obvious warnings reported by checkpatch in all files
except orinoco.c. Orinoco.c is part of different patch series of Dave.

This fixes all errors. Remaining warnings are:

  - one typedef struct. This needs separate patch.

  - one typedef enum. I am not convinced this needs changing

  - long lines where it is not obvious that fixing them improves
    anything.
