>From the log, it looks like firmware can't be ready in time. Maybe you can try to enlarge wait time to see if more time is needed for firmware to be ready.

Card is removed then reinserted. "WLAN FW already running! Skip FW dnld" means driver skips firmware download under this case. It is hard to say if firmware is still all right.
