In the logs you attached to the bugzilla.kernel.org report, the keyboard and mouse were attached to the OHCI controller at 0000:06:00.0. Is that the PCI controller? If you collect a usbmon trace for that bus (bus 4 in the attached logs), what does it show when the problem occurs? If you run a kernel that has CONFIG_USB_DEBUG enabled, what shows up in the dmesg log when the problem occurs?
