[PATCH v6] usb: phy: add R-Car USB phy driver

This patch adds Renesas R-Car USB phy driver.
It supports R8A7779 chip at this point.

R-Car has some USB controllers, but has only one phy-initializer.
So, this driver is counting users.

 But as I explained before, this driver can't use devm_request_and_ioremap(),
 because this Renesas R-Car PHY address has strange mapping.
 Some registers of this PHY address are mapped to crevice of USB EHCI/OHCI register area.
 like below, so I added /* CAUTION */ to explain why it didn't use devm_request_and_ioremap()
