It looks like something is wrong with the hardware, either in the keyboard or in the connecting cable. Can you post a usbmon trace showing what happens when the keyboard is plugged in? Instructions are in the kernel source file Documentation/usb/usbmon.txt. According to your log, you'll need to trace bus 4.
