pull-request: xHCI port array creation bug fix

Hi Greg,

It looks like I introduced two bugs with the port array patch for the
case where there's a broken xHCI host controller.  Fortunately, the bug
won't show up on the NEC host controllers that are out there.  Here's a
patch to fix the bugs.  This patch should also be pushed to the stable
trees that the port array and port disabling patches made it into.  The
patch will be sent out to the list.

I apologize for asking you to pull the original patch, it was too big
for that late in the rc cycle.
