[PATCH 2/4] option: add UTStarcom device IDs for QCDM-capable ports

These devices typically provide a standard CDC-ACM interface which accepts
normal CDMA AT commands.  They also provide an additional serial port
which uses the proprietary Qualcomm Diagnostic Monitor protocol which is
what is getting added here.  The QCDM port can be used to retrieve signal
strength and radio status while the CDC-ACM port is connected with PPP.
