This is likely a bug somewhere in the filesystem or block layers. When a USB storage device is removed and then re-plugged, the kernel cannot be certain that the device that was added was the same as the device that was removed. It looks from this that the filesystem that is still mounted needs to be more cleanly disconnected from the underlying storage device when that device is removed.
