What version of the linux kernel are you using?  You should be using
2.6.29-rc6 or later as this fixes several problems with resetting radio
controllers and wusb hosts after errors.

You need to find out why the HWA rejected this control transfer.  I
would hazard that the HWA firmware has died given all the subsequent errors.
