[PATCH] iser: limit number of CQ entries requested

Hi Tomo,

Sending a patch to limit the number of CQ entries requested to a reasonable
value instead of copying the value from the IB device attributes.
Some versions of IB stack report a very large number that may lead to
consumption of unnecessary resources or even failure to create CQ.
