pulldown are each okay. However, please clearly indicate to the user what is expected such as please check or please select your desired answer. I also believe that check boxes which allow an "x" to appear when checked are easier to discern in lynx or other text-oriented browser. If you allow a user to manually type in their answers, then please be certain that those edit boxes are clearly labeled/tagged such that a Screen Reader can speak what these edit boxes are when the user comes to sed edit box. Many times, I have to literally go into Screen Review in a DOS Screen Reader to verify that I entered information into the appropriate edit box. This is more difficult to do in Windows using a Windows-based Screen Reader. Just my two centes (U.S.) worth!
