list_debug, pcmcia: License cleanup

As you might know we are working on cleaning up the licensing mess in the
kernel and convert it to SPDX license identifiers as the only source of
license information.

Archaeology found unspecific GPL license references, which have been
authored by you.

1) released under the general public license (gpl)

   lib/list_debug.c


2) this code is released under the gnu general public license (gpl) this
   software is provided as is and any express or implied warranties
   including but not limited to the implied warranties of merchantability
   and fitness for a particular purpose are disclaimed in no event shall
   the author be liable for any direct indirect incidental special
   exemplary or consequential damages (including but not limited to
   procurement of substitute goods or services loss of use data or profits
   or business interruption) however caused and on any theory of liability
   whether in contract strict liability or tort (including negligence or
   otherwise) arising in any way out of the use of this software even if
   advised of the possibility of such damage

Can you please either send cleanup patches for the affected files or
indicate which GPLv2 variant you had in mind and I run it through my
cleanup machinery.
