This either indicates a bug in the ext4 journalling layer or that there's some type of hardware problem like memory corruption. Hopefully the fs people can provide more insight. It looks like kswapd is trying to do direct reclaim and the write fails for some reason (which could be the HW problem) and then it doesn't recover from the error.
