[PATCHv3 1/1] ibmvfc: IBM Power Virtual Fibre Channel Adapter Client Driver

This patch adds a new device driver to support the Virtual Fibre Channel
interface on IBM Power based servers. The Virtual I/O Server on IBM Power
servers utilizes N-Port ID Virtualization to export a Virtual Fibre Channel
adapter to the client. This driver is the client device driver.

This driver is now feature complete and has no known outstanding issues, so
it should be ready to merge. Any chance of this getting pulled into 2.6.27?

Changes since last post:

1. Changed some logging code formatting
2. Changed DRC name getting initialized in NPIV Login to use the expected data
3. Removed some debug logging code

Bugs Fixed:

1. Fixed bit defines for TMF ops so that TMFs now work correctly.
