[PATCH 1/5] lpfc 8.2.7 : Change device reset behavior

Change device reset behavior. 
  Prior handler was only waiting for I/O on one lun to finish before
  returning completion. Now, wait for all LUNs on the target.
  Also performed some rudimentary cleanup while in this code.
