[PATCH 133/141] usb: Fix fall-through warnings for Clang

In preparation to enable -Wimplicit-fallthrough for Clang, fix multiple
warnings by explicitly adding multiple break/return/fallthrough
statements instead of letting the code fall through to the next
case.
