various esp_scsi cleanups V2

Mostly to avoid methods calls for dma mapping, but also to tidy up
a few bits found while doing that.

Changes since v1:
 - fix a sun_esp compiler failure in an intermediate patch
 - drop the dev argument to scsi_esp_register
