mpt3sas remove device erros

Hi,

We have ssd failure sometimes, and mpt3sas remove the devices
automatically, and readd again.

For example

I guess mpt3sas somehow detected sdo misbehave, then remove the disk,
and then some how rediscover the devices as sdaa, but with lba out of
range error.

Could some one tell me if that's expected behavior, which part of the
code handle it?
