remove obsolete drivers and scsi_module.c V2

Hi all,

this series removes the obsolete scsi_module.c infratructure that
has been obsolete for 15 years, and the remaining drivers that depend
on them.

Changes since V1:
 - drop already merged patches
 - convert mvme147 to new-style probing instead of removing it
