Hi Doug and Jason,

Here are some general code cleanups and improvements to target the next release.
Nothing too scary here. Changes are limited to hfi1 and one to rdmavt.

Changes since v1:
* Reworked patch to move rhf_offset from dev to ctx data per Jason comment
* Added an additional patch to address the diffhunk that didn't belong
  in the previous patch
