[PATCH] libpvrdma: Fix up for ABI file

Reverted the ABI file to the kernel version. I added a new file that
includes this ABI file and duplicates only the required structures
for the library. This should be temporary until the UAPI is fixed
properly within libibverbs and the kernel.
