It took me a lot of time to reproduce this problem. And finally I can reproduce this problem.

The root cause is:

In this function rxe_create_qp, if some problem occur in rxe_qp_init_req, rxe_init_task functions are not executed. But error handler of rxe_create_qp still calls rxe_cleanup_task. This problem will occur.
