[PATCH] parisc: Fix set_fixmap() on PA1.x CPUs

Fix a kernel crash which happens on PA1.x CPUs while initializing the
FTRACE/KPROBE breakpoints.  The PTE table entries for the fixmap area
were not created correctly.
