[PATCH 14/24] ia64/sba_iommu: improve internal map_page users

Remove the odd sba_{un,}map_single_attrs wrappers, check errors
everywhere, and remove the completly bogus alloc_pages_node call that
uses the dma attributes argument as the node id.
