[PATCH 3/9] fscache,netfs: rename netfs_io_terminated_t as fscache_io_terminated_t

Rename netfs_io_terminated_t as fscache_io_terminated_t to make raw
fscache APIs more neutral independent on libnetfs.

This is a cleanup without logic change.
