[PATCH 5/7] NFS: Use NFSDBG_FILE for all fops

Clean up: some fops use NFSDBG_FILE, some use NFSDBG_VFS.  Let's use
NFSDBG_FILE for all fops, and consistently report file names instead
of inode numbers.
