[PATCH] conversion to bio batch completion

Hi Jens,

This patch is for linux-next, applies cleanly on for-next branch.

This patch contains the following:
	* changes for conversion to bio batch completion from Kent
	* fix to apply the above changes cleanly on latest mtip32xx code
	* batch bio completion changes in 
		* mtip_command_cleanup()
		* mtip_timeout_function()
		* mtip_handle_tfe()
