So we've added yet another bisection hole in there somewhere. And the bisection indicates that this (drop-dead show-stopping box-killing) regression was added by 6e736be7f282fff705db7c34a15313281b372a76 ("block: make ioc get/put interface more conventional and fix race on alloction").
